1095-B Form Available for Current and Previous Staff

This message is to inform eligible staff (with LGHIB benefits) that the Affordable Care Act (ACA) Form 1095 is available upon request.

The Form 1095 provides information about health coverage offered to you during the previous calendar year and may be used when preparing your tax return. You are not required to submit Form 1095 with your tax filing, but you have the option to keep it for your records.
